Nominee must hold a job title of manager level or above at a healthcare organization or in a healthcare-related role in North America. Exception is Rising Stars, who have been in a healthcare career for 5 years or less.
We are looking for achievements over the past 12-18 months. Exception is the Legacy Award which recognizes lifetime achievement.
Your entry should not exceed 1,000 words Supporting materials, including testimonials, are recommended but should not exceed 5 additional documents.
The price of each primary entry is $425. If a secondary entry of the same campaign into any additional categories is submitted, the price will be $275 for each additional category. A late entry fee of $275 per entry for entries submitted after December 4, 2018.
Payment in full must accompany the entry. Entry fees are not refundable.
